This form is to be completed by VicSuper employers who wish to query their new insurance category rating.

Under our new insurance design, as an employer, you will have been provided one of three Insurance Category ratings: High Risk, Medium Risk or Low Risk.

Please note that regardless of the Employer Insurance Category rating provided, individuals can complete the Individual Insurance Category rating form available at VicSuper/forms to obtain an accurate insurance category rating for themselves.

To enable us to determine whether or not your insurance category rating should change you will need to complete the below form and submit it to our team for review.
